❌ Tag it as ‘Failed’ and remove it from service.
❌ Provide a detailed report outlining the failure.
❌ Advise on repairs or replacement to bring it back into compliance.

For workplaces under WorkSafe NZ safety requirements, failed equipment must not be used until repaired or replaced.
